Player Removal

This form is to be used for the removal of any player you no longer wish to be registered for your club and is not being transferred to another team. If the player is being transferred then a transfer request from must be submitted. This form is not to be used if a player is being removed under the maximum squad size rule.

By submitting this form I agree that

The player will be removed no sooner than 7 days after the submission of this form

If this player is registered for the league again there will be a registration charge

The player and their parents are aware they are being removed from the team.

This player will no longer be able to play in any league/cup games oraganised by the Midsomer Norton Youth league, unless the are submitted for registration.
